Nomination for Debenture Membership

The supplementary documentation will need to be uploaded near the end of the application.

  • Copies of passport and HKID card of the main applicant, and the spouse/domestic partner applicant (if applicable).
  • Copy of marriage certificate if the main applicant is married or signed Partner Declaration. Please click here to download the Partner Declaration Form.
  • Copy of documentary proof for the parental relationship between the main applicant / the spouse/domestic partner applicant and each of the child who is single and aged under 25 (if applicable).
  • 1 passport size recent colour photograph (must be with clear background and full frontal face) of each of the main applicant, the spouse / domestic partner applicant and children (if applicable).
  • Business card of the main applicant or letter of employment confirmation issued by the Debenture Holder.
  • Cheque made payable to “Ladies’ Recreation Club” issued by the Debenture Holder for re-nomination fee, if applicable as per conditions of the debenture.
  • Direct Debit Authorisation Form. Please click here to download the Direct Debit Authorization Form.
  • Signature(s) of Applicant(s).
  • Authorized signatory of the Debenture Holder.
  • Company Chop.

 

Conditions:

  1. The main applicant should be an employee of the Debenture Holder.
  2. Full monthly subscriptions are due and payable for the month in which the nominee signs up for membership.
  3. One calendar month’s notice in writing is given to the Club in the event this nomination is terminated.
  4. The Debenture Holder is liable to all outstanding amount accrued in the Debenture Membership account of the nominee. The Club serves its right to withhold any request of the Debenture Holder until the outstanding amount is settled, including but not limited to the nomination of new nominee to the Debenture Membership.
  5. The main applicant and his/her family members (if applicable) nominated for the Debenture Membership will be required to be balloted. The Club does not warrant that the main applicant and his/her family members (if applicable) will be automatically elected by the Balloting Committee.

 

All personal data is protected under the Personal Data (Privacy) Ordinance (CAP.486). The Ladies Recreation Club may use the collected data for verifying your identity, for statistical research and analysis, for designing and marketing the Club's facilities, products and services, and for facilitating the communications between you and the Club. Your personal data supplied to the Club will be kept confidential but some collected data may be accessible by the Club's authorised officers or committees who are obliged to the same confidentiality duty.
